Appointment Instructions

Steps to Prepare for your Appointment:

  1. Fill out and complete your MedXpress on-line history before your appointment. Be prepared to provide your MedXpress number upon request.

  2. Review information and potentially initially disqualifying criteria related to medical conditions for which the FAA has published special requirements and protocols.

Please bring the following to your appointment:

  1. A printed copy of your MedExpress Summary.
  2. Glasses or contacts if you wear them. If you wear contacts, bring whatever you need to take them out. I have to check your uncorrected vision, too.
  3. Your driver’s license or another form of photo ID with an address on it.
  4. Your previous Airman’s Medical Certificate if you have one.
  5. Check or cash. Please see the fee chart listed below for rates.
    • 3rd Class: $150
    • 2nd Class: $175
    • CACI: $50
  6. Any current medications you take to verify doses.
  7. A full bladder for a urine specimen to check for protein and glucose.

Upon Arrival/At the time of your appointment:
