Appointment Instructions
Steps to Prepare for your Appointment:
- Fill out and complete your MedXpress on-line history before your appointment. Be prepared to provide your MedXpress number upon request.

- Review information and potentially initially disqualifying criteria related to medical conditions for which the FAA has published special requirements and protocols.
- Review the FAQ page that covers additional general and medical condition FAQs and information, if needed.

Please bring the following to your appointment:
- A printed copy of your MedExpress Summary.
- Glasses or contacts if you wear them. If you wear contacts, bring whatever you need to take them out. I have to check your uncorrected vision, too.
- Your driver’s license or another form of photo ID with an address on it.
- Your previous Airman’s Medical Certificate if you have one.
- Check or cash. Please see the fee chart listed below for rates.
- 3rd Class: $150
- 2nd Class: $175
- CACI: $50
- Any current medications you take to verify doses.
- A full bladder for a urine specimen to check for protein and glucose.
Upon Arrival/At the time of your appointment:
